The Information We Collect

We need to collect personal information about you and your health in order to give you the best possible treatment. In effect you (requesting treatment) and us (providing treatment) constitutes a contract, and although you can refuse to provide information, we may not be able to treat as a result. Our legitimate interest is so that we can do our job safely, Your legitimate interest is that we may contact you to confirm appointments or provide you updates or information relevant to your medical care.

The Information WE DO NOT Collect

Credit/Debit Card or bank information.

The Way We Use Information

I have a legal obligation with my professional body to retain your records for 7 years after your most recent appointment (or age 25, if this is longer). After this period, your records will be shredded and destroyed.

Your records are stored on paper, in locked filing cabinets and the door is always locked out of working hours.

Your phone number is stored on a mobile phone with a passcode. Any email correspondence will be printed and added to your paper records before being deleted from the computer.

We will never share your data with anyone who does not need access without your consent.  Any third party correspondence (for example with your doctor or other medical practitioner) will be stored in a password protected file electronically and a copy printed and added to your paper records. The electronic file will be removed from the computer when we are satisfied the prevailing issue has been dealt with.

To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect both in the clinic and online, in accordance with GDPR guidelines.

How you can Access or Correct your Information

You can access all your personally identifiable information that we collect online and maintain by calling or sending us an email. We use this procedure to better safeguard your information.
You can correct factual errors in your personally identifiable information by sending us a request that credibly shows error.

To protect your privacy and security, we will also take reasonable steps to verify your identity before granting access or making corrections.
